A support plan built around you


OdysseyCare provides both Support Coordination and Specialist Support Coordination, ensuring that every participant receives the right level of support at the right time.


Our support coordinators take the time to truly understand your situation. We do not offer a one-size-fits-all approach. Instead, we develop a personalised support plan that reflects your goals, your strengths, your circumstances, and the life you want to build.


Whether you need help understanding and implementing your NDIS plan, connecting with the right service providers, or preparing for a plan review, our team brings local knowledge, genuine care, and real expertise to every interaction.

NDIS funding for Support Coordination


Support Coordination and Specialist Support Coordination are both funded through the Capacity Building budget in your NDIS plan, under the Support Coordination support category.


Our team handles all documentation and works closely with participants, families, and other service providers to ensure your services are properly registered and claimed through the NDIS. We make the process as smooth and straightforward as possible, whether your plan is self-managed, plan-managed, or NDIA-managed.

What to know before getting started


To help us deliver the best possible support coordination experience, please keep the following in mind:

  • Please have a copy of your current NDIS plan available when you contact us, as this helps our team understand your funding and goals

  • All participants must complete onboarding documentation prior to services commencing

  • Our team will guide you through all required documentation, including your service agreement, intake form, and consent forms

  • If your circumstances or support needs change at any time, we ask that you let us know as soon as you are able so we can respond and adjust supports promptly

Support Coordination services we provide


Our NDIS support coordination services are comprehensive, flexible, and designed to assist participants across a wide range of needs and situations.


Supports available through our team include:

  • Understanding and implementing your NDIS plan - helping you make sense of your funding categories and what supports you can access

  • Coordinating providers and supports - connecting you with suitable service providers, managing service agreements, and coordinating multiple providers seamlessly

  • Advocacy with the NDIA and mainstream services - advocating strongly on your behalf with the NDIA, health services, education systems, and community services

  • Plan review preparation and support - working closely with you to document progress, identify changing needs, and prepare confidently

Specialist Support Coordination, available at Level 3, is designed for participants with more complex needs, including mental health challenges, crisis situations, housing instability, justice involvement, or high-risk circumstances. It involves a higher level of expertise and intensive coordination across multiple systems and services.

NDIS funded support

Our community nursing services can be accessed through an NDIS plan, depending on participant goals, medical condition, and funding categories.

NDIS community nursing supports may be funded under:

  • Daily Activities (Assistance with Daily Life)

  • Improved Health and Wellbeing

  • Improved Daily Living

  • Support Coordination

  • High Intensity Daily Personal Activities
     

Our team works closely with each participant, their NDIS planner, Support Coordinators, and family to ensure the right support is in place and funding is utilised effectively within their NDIS plan.

Important information

To support safe and effective community nursing care:

  • Please share all relevant medical history, diagnoses, and current treatment plans prior to commencement

  • A nursing assessment will be completed to develop an individualised care plan

  • All onboarding documentation must be completed before nursing supports begin

  • Any changes to a participant's medical condition or treatment should be communicated to our team promptly

  • Our team will liaise with GPs, specialists, and other healthcare providers as required
